MDS Coordinator

Seeking a full-time MDS Coordinator to work with our nursing team in our new building which provides, Long Term Care, Transitional Rehab, and Memory Support services and care. Specific requirements and tasks include:


  • Graduate of an approved RN program and RN licensed in NC
  • Minimum 2 years nursing and MDS experience required
  • Strong knowledge of Case-Mix, the Federal PPS process, and Medicaid reimbursement
  • Thorough understanding of the Quality Measure process
  • Knowledge of MDS 3.0 and exp. in a SNF preferred
  • RAC-CT preferred

Rehab Nurse Manager

  • Full-time

Seeking an RN Manager with experience in orthopedic rehab care to oversee the delivery of services in our new Healthcare Building which contains a rehab neighborhood specializing in orthopedic care.


  • Ensures that patient care requirements are met by scheduling and assigning staff; monitoring attendance, and taking appropriate action
  • Ensures that individual, changing needs of individuals are met by establishing resident care goals, planning and organizing delivery of nursing services according to individual care plans, observing subordinate nursing staff, and implementing interventio
  • Establishes a compassionate environment by developing and maintaining positive relationships with and providing support to staff, residents, and residents’ caregivers through direct interaction and telephone contact as needed
  • Assures quality of care by enforcing adherence to state nursing requirements as well as DFS regulations, monitoring changes in resident condition, communicating changes in resident condition in a timely manner to the resident’s physician(s) and caregiver
  • Maintains nursing staff job results by planning, monitoring, and appraising job results, establishing training goals to improve results, conducting and directing training to meet those goals, assessing effectiveness of training and in-service programs, t

  • Full time - 11:00 a.m. - 7:00 p.m.
  • Part-time - 7:00 a.m. - 3:00 p.m. (3 days per week)

2-3 years professional restaurant/food service industry experience in a high volume establishment; experience in saute, grill, broiler, sauces, soup making and baking skills; catering experience helpful.

Must have good time management and customer service skills; teamwork and good work ethic; ability to use good judgment; must be safety conscious; serve safe certification helpful.

Transports independent living residents in a timely, safe, and courteous manner to activities that occur away from the facility; assists residents getting on/off the vehicle; ensure all supplies needed for an event are transported with residents; ensure that mini vans and bus are kept in good working order; maintains cleanliness of van/bus; delivers campus mail to independent living residents; may also drive residents to medical appointments.


  • Valid driver's license; CDL with P endorsement; 10 years good driving record
  • Good communication and time management skills; customer service oriented; self directed; safety conscious; technical knowledge; flexibility; computer skills


Encourage the independent performance of residents activities of daily living (ADLs) to the extent each individual's condition permits and provides assistance with residents ADLs as needed (feeding, dressing, bathing, mouth care, etc); Supports individual care plans; follows care plans; reports/documents status changes; Uses PPE and follows infection control procedures; Keeps skills up to date by attending mandatory in-services and maintaining C.N.A. certification by pursuing continuing education

  • Licensed RN in state of NC
