Getting on the Priority List
Salemtowne maintains an active priority list for prospective
Salemtowne residents who want a little more time before moving
onto campus. To secure a place on Salemtowne's priority list,
prospective residents must complete an application, financial
disclosure and health history forms. We will also ask when you
want to be contacted and what type of residence interests
you. Then simply submit these forms along with an application
fee. The non-refundable, non-applicable
application fee is $250 for individuals and $350 for couples. No application fee is required for the Assisted Living Center or the Health Care Center.
When
your name comes up on the priority list, a Salemtowne representative will
contact you to ask if you wish to proceed with the application process. Prospective
residents on the list have priority over those not on the list. If
you are not ready to move in when the space is offered, just
tell us and we'll change your move-in interest date without penalty. You can also change your move-in interest
date with a simple phone call or letter to the admissions
office.
By placing your name on the priority list you are not guaranteed
a residence, but rather you reserve the option to apply for
admission when space becomes available. Admission to Salemtowne
depends upon successful completion of the application process. Individuals on the Salemtowne priority list will be placed on our mailing list and will be invited to community events.
Application Process
All applicants for residency must complete an application. The application consists of four parts: one's
personal and family history, a personal health history, a medical form
(completed by the applicant's physician) and a confidential
financial disclosure.
Once a completed application has been
submitted, the materials are reviewed to determine financial viability and to confirm the
appropriate level of residency (i.e. Independent, Assisted or Health Care). To clarify any questions
as to the appropriate level of residency, a medical assessment
will be scheduled with Salemtowne nursing staff.
Applications are reviewed by the admissions committee. The committee considers the application by
assessing the individual's (or couple's) total needs and
Salemtowne's ability to meet those needs. Generally, the
following guidelines are considered:
- All persons are eligible for admission regardless of race,
sex, national origin or religious preference.
- Applicants must be 62 years of age or older. For couples,
one person must be at least 62 or older.
- Applicants to independent living must be able to live independently
without supervision.
- Applicants who cannot live independently will be evaluated
for admission to the Masten Assisted Living Center or Phillips
Health Care Center.
- Applicants should be able to meet the cost of care for
their lifetime. (In certain cases financial assistance may be available. Contact our Admissions office to discuss.)
